The Community-HOPE (Community Health, Outreach, Prevention & Education) Program

The Community Health, Outreach, Prevention, and Education Program (Community-HOPE) is an umbrella for several programs that provide community outreach to the African American communities of Alameda, Contra Costa and Solano Counties.  Our Program includes:

The African American Alzheimer’s Outreach Project

The African American Alzheimer’s Outreach Project is a culturally responsive outreach program designed to support African American individuals and families impacted by Alzheimer’s Disease.  The program provides education, early detection resources, caregiver support, and connections to medical and community services.  Recognizing the disproportionate impact of Alzheimer’s Disease in the African American community, our Project partners with local churches, community centers and health professionals to build trust and awareness.  Through community health fairs, workshops, support groups and culturally tailored materials, the Program empowers families to navigate the challenges of Alzheimer’s Disease with knowledge, compassion, and community strength.

Real Talk:  Erasing The Stigma Of Opioid Use In The African American Community

The Council’s “Real Talk” series provides an opportunity for the Black community to learn to identify the signs and symptoms of opioid dependence; educate our community on the nature of opioid drugs and the treatment options that may be available; reduce the stigma associated with opioid and substance use disorder; provide access to Medication-Assisted Treatment and other Substance Use Disorder programs; and increase awareness of the medications used in these programs.

Mental Health Mondays

Each Mental Health Mondays (MHM) session is led by a mental health professional who navigates a discussion on a variety of subjects including opioid use disorders, depression, bipolar disorder, how to find a therapist, etc.  These sessions have proven to be extraordinarily successful, with participants calling in from the Bay Area, throughout California, other states and internationally.  

MHM addresses the unique mental health, social and emotional challenges faced by the Black Community.  We examine the impact of systemic racism, historical trauma, and underrepresentation in traditional mental health services.  By leveraging this virtual platform, we have been able to provide a convenient and stigma-reducing resource that builds resilience in the Black Community. 

MHM works to dismantle barriers to care, promote healing, and contribute to the broader goal of mental health equity and decreased use of opioids in our community.  Through accessible, culturally competent, and stigma-reducing resources, the program empowers individuals, advance community resilience, and improve the overall well-being of the Black Community.
