Community-HOPE
The Community Health, Outreach, Prevention and Education Program (Community-HOPE) is an umbrella for several programs that provide community outreach to the African American communities of Alameda, Contra Costa and Solano Counties. Our Program includes:

Elevate Youth California
Elevate Youth California (EYC) is a program of the California Department of Health Care Services (DHCS) funded through Proposition 64, the California Cannabis Tax Fund. Sierra Health Foundation’s Center for Health Program Management is contracted to support the implementation of EYC. The Council’s Elevate Youth California Program supports youth leadership development in Oakland and Vallejo to reduce youth substance use disorder (SUD).

Evaluation Services
Urban Strategies Council offers culturally-responsive evaluation services to non-profit organizations that focus on serving the African American and Latino Communities of the East Bay. As part of a larger organization dedicated to equity and health justice, we partner with service-based groups to assess program impact, improve service delivery, and amplify community voices.
